This position offers the flexibility of a remote work setting while being integral to our hospice care facility. As a social worker within this organization, you will have the opportunity to connect with clients from the comfort of their homes, tailoring your support to meet their unique needs. Our work hours are Monday to Friday from 8:00 AM to 4:30 PM, with occasional evening or weekend hours expected based on client needs, ensuring that we adapt to the changing circumstances of those we serve.

• Provide direct clinical services to assigned clients, utilizing evidence-based practices to assess, diagnose, and treat individuals with major mental illnesses and emotional disorders.
• Engage in collaborative case management activities, working alongside other healthcare professionals to form a cohesive approach to each client's care.
• Maintain effective communication with clients, their families, and support systems while respecting diverse backgrounds and traditions.
• Ensure compliance with professional ethical guidelines and policies established by Nurture Mind and Soul, upholding the values of integrity, compassion, and professionalism.
• Actively participate in ongoing professional development to continuously refine skills and drive better outcomes for clients.

This role focuses heavily on supporting individuals who are often experiencing life's most challenging moments. As a Clinical Social Worker, you will engage with clients facing terminal illnesses, psychosocial stressors, and mental health challenges associated with end-of-life care. Your empathy and clinical expertise will empower clients and their families, fostering resilience and improving their overall quality of life during a critical time.

• A Bachelor’s Degree in Social Work or a related field is required.
• Valid LCSW licensure in Nevada, with the ability to function effectively in remote care settings.
• Expertise in assessment, diagnosis, crisis management, and intervention strategies tailored to high-need populations.
• Strong organizational and planning skills to manage a varied caseload efficiently.
• Excellent verbal and written communication abilities to facilitate effective interactions with clients and multidisciplinary teams.

• A minimum of two years of experience in a clinical setting is preferred, especially working with hospice or end-of-life care populations.
• Demonstrated experience in developing treatment plans and delivering solutions to complex emotional and mental health challenges.
• Familiarity with electronic health record (EHR) systems to ensure accurate documentation and streamlined care coordination.
