Care Management Support Coordinator II

Contract Type / Duration: 3-Month Contract

Location: Remote — Nationally Sourced, CST Hours

Work Hours: 8:00 AM–5:00 PM CST, Monday–Friday, with a 1-hour lunch

Pay Rate: $22/hour, based on experience

Job Summary

We are seeking a Care Management Support Coordinator II to support care management activities and help ensure members receive appropriate services and continuity of care. This is a high-volume, member-facing call center role focused on conducting Health Needs Assessments (HNAs) through outbound and inbound calls.

The position requires approximately 50–70 outbound calls per day, strong communication and documentation skills, and the ability to manage multiple tasks in a fast-paced healthcare environment.

Key Responsibilities
  • Conduct outbound and inbound calls with members to complete Health Needs Assessments.

  • Engage members regarding their care needs, services, resources, questions, and next steps.

  • Coordinate care activities with care managers, healthcare providers, community organizations, members, and caregivers.

  • Identify member needs and make appropriate referrals to Care Managers, Disease Managers, and community-based organizations.

  • Provide education regarding available healthcare benefits and resources.

  • Support member and provider inquiries related to care plans and services.

  • Document member interactions, assessments, care needs, and activities accurately.

  • Maintain member records in accordance with applicable requirements and guidelines.

  • Communicate with care managers, practitioners, and other team members to support continuity of care.

  • Work effectively in a high-volume environment while meeting productivity and quality expectations.

Top Required Skills
  1. High-volume healthcare customer service experience — Strong phone communication, documentation, multitasking, and member engagement skills.

  2. Microsoft Office and technical proficiency — Working knowledge of Excel, Word, Outlook, and remote-work technology; comfortable working with dual monitors.

  3. Managed care or healthcare experience — 2–3 years of managed care, health insurance, or relevant healthcare/physician office experience, preferably with Medicare/Medicaid.

Performance Expectations

  • Maintain a 90% or higher call audit score.

  • Maintain a 90% or higher Health Needs Assessment audit score.

  • Meet established screening and productivity requirements.

Training

Training is approximately 3½ weeks, Monday–Friday, 8:00 AM–5:00 PM CST, with a 1-hour lunch.

Training requires cameras to be on, and candidates should be prepared for a full-time training schedule without time off during the training period.
