About the Role

We are seeking a compassionate and motivated Pediatric Physical Therapist to join our growing home health team. In this role, you will provide individualized physical therapy services to pediatric patients in their homes, helping them improve mobility, strength, coordination, and overall functional independence.
This is an excellent opportunity for a therapist who values autonomy, flexibility, and meaningful one-on-one care in a home-based setting.
Responsibilities

  • Perform initial evaluations and develop individualized treatment plans for pediatric patients
  • Provide skilled physical therapy interventions in accordance with physician orders and plan of care
  • Assess patient progress and modify treatment plans as needed
  • Educate patients and caregivers on therapeutic techniques and home exercise programs
  • Maintain accurate and timely clinical documentation
  • Collaborate with interdisciplinary care team including OT, ST, nursing, and physicians
  • Ensure compliance with agency policies and state/federal regulations
Qualifications

  • Graduate of an accredited Physical Therapy program
  • Current Texas Physical Therapist License (or eligible)
  • Pediatric experience preferred (home health experience a plus)
  • Strong clinical decision-making and communication skills
  • Reliable transportation and valid driverโ€™s license
  • Current CPR Certification
