  • Collaborate with the integrated clinical team to design and implement a comprehensive, evidence-based virtual-first cardiac recovery program.
  • Regularly review and re-evaluate treatment plans to ensure they align with best practices and adapt as necessary for maximum patient benefit.
  • Promote patient engagement through effective counseling strategies, including behavioral change techniques such as mindfulness and motivational interviewing.
  • Maintain clear communication with both the clinical team and partner health systems, addressing any unusual or emergent situations promptly.
  • Provide thorough and accurate documentation of all patient interactions and session outcomes to ensure continuity and efficacy of care.
