• Deliver virtual psychotherapy sessions, each lasting 45-50 minutes, to a diverse panel of patients coping with various behavioral health challenges.
• Conduct comprehensive evaluations of patient symptoms and risk factors to accurately determine diagnoses and create tailored treatment plans that address individual needs.
• Collaborate with the integrated clinical team to design and implement a comprehensive, evidence-based virtual-first cardiac recovery program.
• Regularly review and re-evaluate treatment plans to ensure they align with best practices and adapt as necessary for maximum patient benefit.
• Promote patient engagement through effective counseling strategies, including behavioral change techniques such as mindfulness and motivational interviewing.
• Maintain clear communication with both the clinical team and partner health systems, addressing any unusual or emergent situations promptly.
• Provide thorough and accurate documentation of all patient interactions and session outcomes to ensure continuity and efficacy of care.

This role focuses on serving individuals across various demographics who are experiencing behavioral health issues, particularly those undergoing detox and cardiac recovery. The diversity of our patient base ranges from young adults to seniors, each presenting unique challenges in their mental health journeys. Your expertise will guide them through their recovery process, facilitating both mental and emotional well-being.

• A Bachelor’s Degree in Social Work is required; a Master’s degree is preferable.
• Current licensure as a Clinical Social Worker (LCSW) is mandatory to practice in New York.
• This role requires an innovative mindset and the ability to navigate a virtual care environment effectively.
• Strong collaborative skills for working within an integrated clinical team are essential, alongside excellent communication abilities.

• Experience working within an integrated healthcare setting or understanding of cardiac rehabilitation is highly preferred.
