As a Licensed Clinical Social Worker at Steadfast Support Clinic, your role involves a wide range of responsibilities, essential for both client care and organizational goals:

• Maintain comprehensive and accurate clinical documentation, including progress notes and treatment records, in alignment with department policies and best practices.
• Collaborate with multidisciplinary team members, crafting tailored treatment and discharge plans based on each client’s psychosocial assessment and predetermined social work intervention methods.
• Engage in consultations with clients' families and support systems, ensuring everyone involved understands the treatment plans and discharge processes, fostering a holistic approach to care.
• Develop and implement individualized treatment plans that adapt to clients’ needs and varying circumstances, adjusting strategies as necessary to optimize outcomes.
• Conduct regular counseling sessions, employing evidence-based therapeutic modalities to support clients in their recovery journey.
• Continuously assess and monitor patient progress towards their goals, making modifications to treatment as needed to ensure effectiveness.
