Work EnvironmentAs a remote-based team member, you will have the flexibility to work from home while remaining integral to our rehabilitation team. Regular business hours are expected, with the possibility of occasional evening and weekend shifts depending on patient needs and assigned caseloads. Key Responsibilities- Conduct comprehensive psychosocial assessments through in-depth interviews, observational techniques, and thorough medical record reviews. This will evaluate emotional, social, and environmental factors influencing patients' recovery progress.
- Provide counseling and psychosocial support to both patients and their families, with a focus on helping them adjust to significant life changes, enhancing their emotional well-being, and improving their coping mechanisms.
- Collaborate closely with an interdisciplinary rehabilitation team to create and implement individualized treatment plans that align with patients' psychosocial needs and recovery goals.
- Assist patients in connecting with essential community resources such as support groups, vocational rehabilitation services, and housing assistance programs, facilitating holistic recovery pathways.
- Offer informative education to patients and their families about available post-rehabilitation services and community resources, empowering them throughout their journey.
- Participate in discharge planning to ensure safe and effective transitions for patients returning home or moving to different care settings.
You will work with a diverse clientele facing a range of challenges, including those recovering from strokes, spinal cord injuries, orthopedic surgeries, neurological disorders, and other impactful conditions. Your role will be pivotal in addressing the emotional and psychosocial challenges these patients encounter, enabling them to reintegrate into their communities successfully.
Qualifications- A Master’s degree in Social Work from an accredited institution is required.
- Active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
- Strong communication and interpersonal skills are essential for effective patient interaction and team collaboration.
- Prior experience in a rehabilitation or medical setting is preferred and will enhance your ability to support patients through their recovery.
