- Provide individual and family counseling sessions to address adjustment difficulties, emotional distress, and the psychosocial dynamics of recovery.
- Collaborate with the rehabilitation team—comprised of medical professionals such as physicians, nurses, and therapists—to design personalized treatment plans that align with each patient’s holistic needs.
- Assist patients in accessing vital community resources, including support groups, vocational rehabilitation services, and housing assistance programs.
- Educate patients and their families on available community resources and facilitate smooth transitions to aftercare, ensuring continuity of support post-rehabilitation.
- Engage in discharge planning, working closely with healthcare providers to create a seamless pathway to home or alternative post-treatment settings that fosters patient independence.
