Work Environment

Regular business hours are required, but there may be occasional evening and weekend shifts to align with patient needs and caseload requirements.

Key Responsibilities

• Conduct comprehensive psychosocial assessments through patient interviews, clinical observations, and detailed reviews of medical history, identifying emotional, social, and environmental factors impacting recovery.
• Provide individual and family counseling sessions to address adjustment difficulties, emotional distress, and the psychosocial dynamics of recovery.
• Collaborate with the rehabilitation team—comprised of medical professionals such as physicians, nurses, and therapists—to design personalized treatment plans that align with each patient’s holistic needs.
• Assist patients in accessing vital community resources, including support groups, vocational rehabilitation services, and housing assistance programs.
• Educate patients and their families on available community resources and facilitate smooth transitions to aftercare, ensuring continuity of support post-rehabilitation.
• Engage in discharge planning, working closely with healthcare providers to create a seamless pathway to home or alternative post-treatment settings that fosters patient independence.

Patient Population / Client Focus

This role is centered around patients undergoing detoxification. The population served often faces multifaceted challenges, including substance addiction and co-occurring mental health conditions. As an LCSW, you will be instrumental in helping these individuals navigate their emotional and social hurdles, ultimately enhancing their capacity for recovery and reintegration into society.

Qualifications

• Master's degree in Social Work from an accredited institution.
• Current state-issued license as a Licensed Clinical Social Worker (LCSW).
• Proven ability to utilize assessment tools and therapeutic interventions effectively.
• Exceptional communication skills, both verbal and written, with a focus on empathy and understanding.

Preferred Experience

• Previous experience in a rehabilitation or medical setting is highly desirable.

Compensation and Benefits
