The American Counseling Center is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team on a full-time basis. In this vital role, you will support patients' emotional and psychological needs as they navigate their rehabilitation journey. You will be working primarily in a remote setting, serving a diverse patient population at our Community Health Clinic in Irvine, California.

• Perform thorough psychosocial assessments by conducting interviews, observing patient behavior, and reviewing medical records to assess emotional, social, and environmental factors that may affect recovery.
• Provide counseling and mental health support to patients and their families, addressing adjustment issues related to illness or disability and improving emotional well-being.
• Work closely with rehabilitation team members to develop personalized treatment plans, incorporating patients' psychosocial needs as essential components of their care strategy.
• Facilitate access to community resources, guiding patients towards support groups, vocational rehabilitation services, and housing assistance to enhance their quality of life post-treatment.
• Educate patients and their families about available community resources and post-rehabilitation services, ensuring successful transitions back to their daily lives.
• Participate in discharge planning and ensure that patients are prepared for life after treatment, including relaying crucial information and support strategies.

In this role, you will engage with a varied patient demographic, ranging from those recovering from physical injuries or surgeries to individuals dealing with chronic illnesses and psychological issues. You will play a crucial role in helping patients regain their independence and reintegrate into their communities. Your work will focus on evaluating each patient's unique set of challenges and empowering them to overcome adverse situations.

Candidates must possess a Bachelor's degree in social work or a related field, along with a current and valid LCSW license. A minimum of one year of experience in a clinical setting is required, demonstrating a solid foundation in social work practices.
