Our center is recognized for its high-quality evidence-based therapies and a strong focus on maximizing functional independence through comprehensive rehabilitation services. We specialize in delivering physical therapy, occupational therapy, and speech-language pathology, with a particular emphasis on conditions affecting mobility and cognitive function such as stroke, spinal cord injuries, and neurological disorders.

  • Provide ongoing counseling and emotional support to patients and their families, guiding them through adjustment challenges while fostering their emotional well-being.
  • Collaborate closely with our multidisciplinary rehabilitation team to develop individualized treatment plans that incorporate the psychosocial needs of each patient into their rehabilitation strategy.
  • Assist patients in navigating community resources, including support groups, vocational rehabilitation options, and housing assistance to promote their post-recovery independence.
  • Offer education and resources to patients and their families regarding available community services and post-rehabilitation support programs.
  • Engage in regular team meetings and case discussions to ensure cohesive care delivery and optimal patient outcomes.
Patient Population / Client Focus

You will primarily work with a diverse patient population experiencing various forms of addiction and emotional distress requiring comprehensive behavioral health support. Your expertise will be pivotal in helping those facing challenges stemming from substance use disorders as well as co-occurring mental health issues. Your role will significantly impact the patients’ journey toward recovery, addressing not only their emotional needs but also equipping them with tools for sustainable success post-treatment.

Qualifications
  • Must possess a current and valid Licensed Clinical Social Worker (LCSW) credential in California.
  • A Bachelor’s Degree in Social Work or a related field is required, with a preference for candidates who have pursued further education.
  • A minimum of 1 year of direct experience providing psychosocial support within a clinical or rehabilitation setting.
Preferred Experience
  • Experience in substance abuse treatment or detox programs, with a strong understanding of recovery-oriented practices.
  • Familiarity with electronic medical record (EMR) systems for documentation and patient management.
  • Proven ability to work effectively in a collaborative environment while managing a caseload of patients.
