The typical work hours are Monday to Friday from 8:00 AM to 4:30 PM, though occasional evening or weekend work may be required depending on client needs. As a Licensed Master Social Worker at Life Stage Counseling, you will have a diverse range of responsibilities, including:

• Conducting comprehensive psychosocial assessments for seniors, encompassing their emotional, mental, and social needs to formulate tailored care plans.
• Collaborating closely with clients and their families to identify personal goals and preferences, ensuring that care plans reflect their wishes.
• Providing ongoing counseling and emotional support to seniors and their families, assisting them in navigating life transitions, such as end-of-life decisions or health crises.
• Advocating for clients’ rights, helping them access community resources, and ensuring they receive the support they require.
• Participating in a multidisciplinary team approach, working alongside nurses, caregivers, and other healthcare professionals to enhance overall client care.
• Managing regular client visits to monitor well-being, evaluate living situations, and address any safety or health concerns.
• Facilitating effective communication among family members to resolve conflicts or concerns in the care of the senior client.
• Educating clients and their families about available resources, support groups, and programs to improve social engagement and well-being.
• Maintaining meticulous documentation of client assessments, progress notes, and care interactions to ensure continuity of care and regulatory compliance.
• Staying current on social work practices and ethical standards, contributing to a proactive approach to client care.

Your primary focus will be on the senior population engaged in hospice care. This role requires a deep understanding of aging-related issues, as you will work intimately with clients coping with various psychosocial challenges compounded by age-related transitions. The ideal candidate will possess the following qualifications:

• A Master's degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Master Social Worker (LMSW).
• A minimum of two years of experience in a similar social work role, preferably within a hospice or geriatric community setting.
• Strong knowledge of social work principles, aging-related issues, and local community resources available for seniors.
• Exceptional assessment, counseling, and problem-solving abilities.
• Ability to demonstrate empathy, compassion, and build strong rapport with seniors and their families.
• Excellent communication skills, with a capacity to collaborate effectively across interdisciplinary teams.
