Key Responsibilities
  • Conduct detailed evaluations and assessments to understand each child's unique needs and development trajectory.
  • Participate in the development and implementation of Individualized Family Service Plans (IFSP), tailoring interventions to meet specific family dynamics and child development goals.
  • Regularly monitor and evaluate outcomes, collaborating with a multidisciplinary team to ensure comprehensive service delivery.
  • Provide essential support to families, offering guidance and resources that promote the emotional and developmental growth of their children.
  • Ensure compliance with all relevant Federal and State regulations governing social work practice in hospice settings.
  • Utilize evidence-based practices and adopt a coaching model to implement interventions in natural learning environments.

Patient Population / Client Focus

In this role, you will primarily work with children under three years of age who have disabilities or developmental delays. Children in our care are at a crucial developmental milestone, and your expertise will greatly aid them in acquiring physical, cognitive, social/emotional, and communication skills. Collaborating with families, you will focus on resilience and empower them through effective strategies tailored to each childโ€™s needs.

Qualifications
  • A Masterโ€™s Degree in Social Work or a Doctorate of Social Work.
  • Active licensure from the Department of Public Health as a Licensed Clinical Social Worker (LCSW).
  • A valid driver's license is required for travel to client homes or other meeting locations as necessary.
  • A minimum of one year of experience in working with children, particularly in early intervention settings.
