This position is remote, allowing for flexibility in managing your work-life balance while serving a vital population. You will primarily collaborate with nursing staff, caregivers, and healthcare professionals at our affiliated hospice center, leveraging modern technology to maintain client contact and support. While the role is conducted remotely, you will regularly engage with clients and coordinate services directly to ensure seamless care delivery.

• Conduct comprehensive assessments of seniors' psychosocial needs, taking into account individual emotional, mental, and social factors.
• Create tailored care plans in collaboration with clients and their families, ensuring that their goals and preferences are clearly articulated and respected.
• Provide individual and family counseling, helping clients navigate life transitions, end-of-life decisions, and complex emotional challenges.
• Advocate for clients' rights, assisting them in accessing community resources and support services tailored to their unique circumstances.
• Regularly visit clients to monitor their well-being and living conditions, addressing any safety concerns or issues that may arise.
• Facilitate effective communication among family members to resolve conflicts and enhance cooperative decision-making regarding care.
• Educate clients and families about available community resources, support groups, and relevant programs to bolster social engagement.
• Maintain meticulous documentation of client interactions, care plans, assessments, and progress notes complying with organizational and regulatory standards.
• Stay current with social work best practices and policies, embodying ethical standards in all client interactions.

Your focus will predominantly be on seniors receiving hospice care, reflecting a sensitive understanding of their physical, emotional, and social challenges. Many of these clients are at critical stages in their lives, requiring dedicated attention to their unique needs as they navigate complex healthcare decisions and personal transitions. The role directly contributes to enhancing their quality of life, emphasizing both emotional support and resource accessibility.

• A Master's Degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Master Social Worker (LMSW).
• A minimum of two years of relevant social work experience, particularly in home care or hospice settings.
• Strong knowledge of psychosocial assessments, aging-related issues, and available community resources.
• Exceptional communication and interpersonal skills, allowing for effective collaboration with clients and interdisciplinary teams.
• Proficiency in electronic health record (EHR) systems and other necessary computer applications for documentation and communication.

• Experience working directly with seniors in a hospice or geriatric setting.
• Familiarity with community resources specific to the Cape Coral area that cater to the elderly population.
• Strong skills in conflict resolution and crisis management.
• Background in advocacy work for vulnerable populations.

We offer a competitive salary range of $60,000 to $100,000 per year, commensurate with experience and qualifications.
