Key Responsibilities
  • Conduct thorough and ongoing assessments to gauge the mental health needs of your clients, ensuring tailored treatment approaches.

  • Develop and provide tentative schedules for clients regarding appointments, home visits, and observations, ensuring a clear pathway to effective care.
  • Coordinate home visits that adhere to organizational standards, allowing for comprehensive evaluations, including chart reviews and staff interviews.
  • Carry out the program plan effectively, monitoring its execution during client sessions and providing feedback for improvement.
  • Participate in Interdisciplinary Team Meetings to discuss client progress and collaborate to enhance treatment approaches.
  • Document and complete monthly and quarterly progress notes to reflect client developments and program effectiveness.
  • Organize training sessions to certify staff and team members in the implementation of the program and adherence to documentation standards.
  • Deliver follow-up interactions such as interviews and additional home visits aimed at ensuring continued support for clients.
  • Act as a liaison with external partners and the broader mental health community to foster collaboration and develop client support networks.
  • Assist in creating procedures that improve training materials for staff, enhancing program outcomes for individuals served.
Patient Population / Client Focus

As a Licensed Clinical Professional Counselor, you will work with a diverse client population, including adults and youth facing various mental health challenges. Qualifications
