Harmony United Care Center is seeking a dedicated and passionate Licensed Clinical Social Worker (LCSW) to join our team in the hospice setting. This position operates remotely, allowing the LCSW to connect with clients through virtual platforms while also engaging with them in their homes and community settings. • Provide individualized clinical assessments and interventions for clients experiencing mental health challenges, specifically focusing on those within a hospice care framework.
• Conduct brief evidence-based screenings and facilitate comprehensive assessments when necessary, adhering to best practices and ethical guidelines.
• Develop tailored care plans that integrate medical, psychological, and social service needs, ensuring seamless communication among clients, families, and interdisciplinary teams.
• Deliver therapeutic services, which include individual therapy, family therapy, and crisis intervention as required, maintaining a caseload that is manageable and diverse.
• Actively participate in team meetings and collaborative care discussions to advance client goals and improve service delivery quality.
• Utilize appropriate referral and resource systems to link clients with additional services and supports.
• Monitor and evaluate client progress, adjusting care plans as needed and ensuring timely documentation and reporting of all client interactions.
• Engage in continuous professional development to enhance clinical skills and stay informed about best practices and emerging trends in social work.

The ideal candidate will work primarily with hospice patients who may experience not only physical health decline but also significant emotional, psychological, and social challenges. Engaging with clients and their families in this sensitive environment requires compassion, cultural competency, and a thorough understanding of grief and end-of-life care. You will develop trusting relationships while assisting clients and their families navigate the complex emotions associated with terminal illness.

• A Master’s degree in Social Work (MSW) from an accredited institution.
• A current license as a Clinical Social Worker (LCSW) in the state of Tennessee.
• Strong assessment, diagnosis, and treatment skills tailored to diverse populations in under-served areas.
• Exceptional communication abilities, both verbal and written, to effectively convey concepts and coordinate care.
• Organizational and planning skills with attention to detail to manage case load efficiently.
• Ability to work collaboratively in a multidisciplinary team environment and demonstrate commitment to cultural competency.

• A minimum of 2 years of clinical experience, preferably in a healthcare or hospice setting.
• Experience handling cases involving grief, loss, and end-of-life issues.
• Familiarity with electronic health record (EHR) systems and telehealth technologies.

The position offers a competitive salary range of USD 70,000 – 85,000 per year, commensurate with experience and qualifications.
