Love Bound Therapy is excited to announce an opening for a Licensed Clinical Social Worker (LCSW) to join our dedicated team, contributing to the well-being of senior clients in a hospice setting. This position allows for flexible working hours with a Monday to Friday schedule from 8:00 AM to 4:30 PM, with some flexibility to accommodate clients' needs, including occasional on-call responsibilities. Though primarily remote, the LCSW will coordinate closely with a hospice team and may conduct site visits as needed. • Conduct thorough psychosocial assessments to understand the emotional and social factors affecting our senior clients, taking into account their mental health needs and living situations.
• Develop personalized care plans in collaboration with clients and their families, setting clear, achievable goals that respect their wishes and preferences.
• Provide counseling and emotional support to seniors facing end-of-life challenges, helping them and their families navigate complex emotions during transitions.
• Serve as an advocate for clients, assisting them in accessing necessary community resources, services, and support systems.
• Work hand-in-hand with a multidisciplinary team, including nurses and healthcare providers, to coordinate care effectively and ensure comprehensive support for clients.
• Perform regular follow-up visits to track clients' well-being, evaluate their living conditions, and proactively address any potential issues.
• Facilitate conflict resolution among family members regarding care decisions, ensuring a harmonious approach to the client's needs.
• Educate clients and their families about community resources, support groups, and tools available to promote engagement and enhance their quality of life.
• Maintain meticulous documentation of client interactions, assessments, care plans, and progress notes to ensure accurate records and compliance with ethical standards.
• Stay updated on social work trends, policies, and best practices to uphold the highest levels of professional integrity and care.

The LCSW will specifically focus on geriatric clients at a hospice center, providing critical support to elderly individuals with complex emotional and psychosocial needs. These clients often face significant life transitions, making this role vital for facilitating their comfort, dignity, and quality of life during challenging times.

• A Master’s degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of professional experience in a social work capacity, ideally within a community center, hospice, or geriatric setting.
• Strong knowledge of aging-related issues and community resources available to seniors and their families.
• Excellent communication, assessment, and counseling skills, combined with a strong ability to empathize with clients from diverse backgrounds.
• Proficiency in digital tools, including the use of electronic health records (EHR) and other software relevant to client care.

While not required, experience working directly with clients in hospice care or with individuals facing terminal illnesses is highly desirable. Familiarity with grief counseling, advanced care planning, and navigating family dynamics in challenging health situations will also enhance a candidate's effectiveness in this role.

The position offers an attractive yearly salary range of USD 60,000 to 100,000, commensurate with experience and qualifications.
