• Act as a primary liaison between clients, families, and outpatient providers to ensure cohesive care.
• Engage with insurance companies to facilitate pre-certification processes and manage utilization for optimal client coverage.
• Participate actively in discharge planning and aftercare strategies that support sustained client wellness post-treatment.
• Provide therapeutic meal support sessions, educating clients on developing healthier relationships with food.
• Deliver compassionate, empathetic, and affirming care tailored to each client's unique needs and intersecting identities.
• Utilize electronic health record (EHR) systems to document progress notes, treatment plans, and client interactions effectively.
• Conduct thorough assessments and develop personalized treatment plans in collaboration with clients.

• A Master’s degree in Social Work from an accredited institution is required.
