Job description

IMMEDIATE HIRE


Hours: Monday through Friday 4 pm - 8 pm

Weekends are flexible for community outings.

Assist the individual in his/her as documented in the ISP, Service Plan, etc.

  • Teach/assist the client in cooking simple meals.
  • Routine hygiene care throughout the day.
  • Medication reminder
  • Supervise in addressing the ISP/Plan of Care goals and document progress
  • Support all opportunities for the growth, learning, and development of the individual
  • Complete daily progress notes highlighting person center events
  • Complete understanding of home policies and procedures including home rules, disaster procedures, participant's rights, etc.
  • Document progress notes before leaving the job site
  • Document tracking of ISP goals before leaving the job site
  • Ensure the individual is integrated into the community
  • Notify the agency of any concerns regarding the individual
  • Attend all staff meetings (required), and participate in scheduled training sessions including on-the-job training
  • Promote harmonious relationships with other staff members
  • Maintain individual's records on-site following policy and procedures
  • Other duties as assigned

MINIMUM QUALIFICATIONS
  • Must be at least 18 years of age or older
  • Substitute/Home Economics Teacher
  • 16 hours of training yearly
  • CPI
  • First Aid certified (required)
  • CPR certified (required)
  • Excellent administrative skills
  • Must be able to read and write English
  • Communicate effectively with service monitors, staff, families, and state administrators
  • Excellent interpersonal skills
  • Resume reflects Direct Support experience
  • Clean 7-year MVR
  • Social Security Card
  • Driver's License
  • Current Physical Exam
  • Current TB test
  • High School Diploma or GED
  • Copy of Professional License
  • Certificates of Training
  • Auto Insurance

Background is required and random drug testing.
