Job description

Primary Care Physician – Senior-Focused Outpatient Practice
Greenville, SC
Salary: $225K-315K

A rapidly expanding, senior-focused outpatient care organization is seeking dedicated Primary Care Physicians who want to practice medicine with purpose. This model is built for physicians who value time with patients, strong team-based support, and a meaningful role in improving the health and well-being of older adults. This is a full-time Monday–Friday position (8am–5pm) with no nights, no weekends, and no inpatient responsibilities.

Why This Role Stands Out

Physicians in this model enjoy a low daily volume, an integrated support team, and the ability to truly focus on quality—not quotas. With a mission-driven culture and a structure that prioritizes patient relationships, clinicians are empowered to deliver the best care possible while maintaining work-life balance.

What You’ll Do

Provide compassionate, comprehensive primary care to a senior population in an outpatient-only setting. Evaluate and treat patients using evidence-based standards. Work closely with a multidisciplinary care team—including NPs/PAs, nurses, behavioral health specialists, social workers, clinical pharmacists, care coordinators, and quality-based coders. Participate in daily huddles, collaborate on care plans, and ensure accurate documentation aligned with quality and value-based standards. Act as a leader within the center by promoting coordinated care, proactive outreach, and exceptional patient experiences.

What Makes This Model Different

Low patient census—typically 12–14 patients per day—allowing you to slow down and truly engage with each patient. Comprehensive care team support so you can focus wholeheartedly on clinical decisions. Centers intentionally designed with seniors in mind and built for comfort, accessibility, and efficiency. A compassionate, team-oriented culture where your voice matters and your work has lasting impact. A highly structured onboarding program ensures you never feel like you’re on your own.

What You Bring

MD or DO with board certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine. Current or attainable medical license in the state of practice. Strong interest in senior care and value-based medicine. Excellent communication and interpersonal skills. Experience in geriatrics, Medicare Advantage, VBC, HEDIS, or EMR systems is helpful—but not required for motivated learners. Candidates must be pursuing full-time, Monday–Friday employment.

Compensation & Benefits

Competitive base salary range of $225,000–$315,000 depending on experience and location. Robust bonus opportunities tied to quality and patient outcomes. Individualized sign-on incentives available. Full benefits package including medical, dental, vision, life, disability, paid time off, CME allowance and time, 401(k) with employer match, malpractice coverage (occurrence-based), and relocation support. Additional professional development support through a structured learning program for all new physicians.

Ideal Candidate Profile

Physicians who thrive in a slower-paced, relationship-centered environment. Clinicians passionate about caring for older adults and improving health outcomes. Strong communicators who embrace teamwork and coordinated care. Providers with ties to the community are encouraged to apply. Experienced physicians and 2025 residents or Geriatric Fellows are welcome—those with Medicare Advantage experience are especially strong fits.
