Position Overview

An established healthcare organization is seeking an experienced Manager of Case Management to support and oversee inpatient case management operations. This leadership role partners closely with the Director of Case Management to ensure effective care coordination, utilization management, discharge planning, and performance improvement initiatives while promoting high-quality patient outcomes and efficient use of healthcare resources.

The ideal candidate is an experienced nursing or social work leader with a strong background in hospital-based case management, utilization management, and multidisciplinary collaboration.

Key Responsibilities
  • Assist in the leadership and daily management of the inpatient case management program.
  • Coordinate day-to-day operations involving care facilitation, utilization management, case management, and discharge planning.
  • Support departmental performance through quality improvement and utilization management initiatives.
  • Analyze financial and quality metrics to identify opportunities for operational improvement.
  • Collaborate with physicians, nursing leadership, social workers, care coordinators, and community partners to improve patient care transitions.
  • Help achieve targeted financial and quality outcomes through effective case management practices.
  • Participate in recruitment, staff development, performance evaluations, and other personnel activities in partnership with department leadership.
  • Lead and mentor a high-performing interdisciplinary team.
  • Utilize data to develop and implement performance improvement strategies.
  • Ensure compliance with organizational policies, regulatory standards, and patient safety initiatives.
  • Promote professional development through education, mentoring, and competency support.
  • Perform additional leadership responsibilities as assigned.
Required Qualifications
  • Bachelor's degree in Nursing (BSN) or Social Work (BSW).
  • Current Texas Registered Nurse (RN) license or Licensed Master Social Worker (LMSW) license.
  • Case Manager Certification required.
  • Minimum five (5) years of experience in utilization management, case management, discharge planning, or another cost/quality management program.
  • Minimum three (3) years of hospital-based nursing or social work experience.
  • Minimum three (3) years of leadership experience.
  • Strong understanding of utilization management, managed care, discharge planning, and care coordination.
  • Excellent communication, collaboration, and relationship-building skills.
Preferred Qualifications
  • Master's degree.
  • Licensed Clinical Social Worker (LCSW).
  • Experience with business planning and performance improvement initiatives.
  • Demonstrated success working collaboratively with physicians and interdisciplinary healthcare teams.
