We are looking for:

Compassionate and dedicated individuals who are passionate about providing top-notch patient care. The ideal candidate will share the same unwavering commitment to excellence, innovation and respect.

What to expect in this role:

You'll provide therapeutic intervention and social work services to patients and their families to enhance comprehensive, integrated and uninterrupted care in the hospital and to facilitate continuity of care in the community. You will conduct patient assessments, develop care plans, coordinate with providers and serve as a patient advocate.

       After 6 months, you'd have the option to work weekends and holidays remotely with equipment provided.

       The Case Management department has recently redesigned into a dyad Social Work/RN Case Manager model where UR is remote, allowing staff to excel in their chosen profession.

EDUCATION/EXPERIENCE REQUIRED:

       Have completed a Master's degree in Social Work.

       Possess an unrestricted LMSW licensure in the state of Michigan.

       Possess a minimum of three (3) years clinical experience.

       Can communicate effectively with patients, families, and hospital personnel, have strong critical thinking and creative problem-solving skills and excel at planning.

       Are self-motivated and enjoy working in a fast-paced environment.

CERTIFICATIONS/LICENSURES REQUIRED:

Licensed Social Worker (LMSW) with a valid, unrestricted State of Michigan license. Certification in Case Management (CCM) by the Commission for Case Management Certification (CCMC) or Accredited Case Manager (ACM) by the American Case Management Association preferred.
