Social Work Transition Navigator - Reliance Community Care Partners

Reliance Community Care Partners (RelianceCCP) has a unique opportunity for a Social Worker or health services professional to take their career to the next level. The Transition Navigator offers flexibility, work from your home office, traveling to participant locations with access to a team of other experienced navigators.

RelianceCCP is a Grand Rapids based, non-profit health care and case management organization, that specializes in coordinating community health care services for older adults and those with disabilities. We work collaboratively with health care providers, government agencies, insurance providers, legal representatives, and family members to help those we serve transition from an institutional setting to the community. RelianceCCP has a turnover rate around 10% for over 15 years.

What Will You Do?

In this community-based program, the Transition Navigator assesses individuals in an institutional setting and assists with overcoming barriers to allow for a safe transition into the community setting of the individual's choice.

Responsibilities

  • Perform in-person assessments to identify the individual's desires, resources, strengths and barriers to transitioning into the community
  • Formulate an individualized plan of care to facilitate the transition
  • Coordinate with other natural supports and professionals to develop, initiate, facilitate and monitor the plan of care
  • Find housing options and coordinate with apartments, assisted living facilities, etc.
  • Conduct or coordinate home safety evaluations
  • Advocate and refer individuals for procurement of services and entitled benefits
  • Follow the individual in the community for as long as the individual is eligible
  • Participate in shopping for various needs
  • Documentation in different software systems
  • Reliable transportation to travel throughout various counties

Education and Experience Requirements

  • BSW or MSW licensure (limited or full) in the State of Michigan (master's preferred); OR

  • A Community Health Worker certification with at least 3 years of experience in the provision of health or social services.
