Patient Coordinator

Starting at $40,000.00 a year depending on experience

We're growing our team and looking for a Patient Coordinator who shares our passion for people, professionalism, and purpose.

What You’ll Do:

As a Patient Coordinator, you’ll play a critical role in the day-to-day operations of our care delivery. You’ll serve as the hub between patients, caregivers, and our internal teams to ensure care is scheduled smoothly and visits are completed with excellence. You’ll also support key administrative functions like billing and payroll prep — helping us keep everything running behind the scenes.

Key Responsibilities:

• Bilingual: Spanish Speaking
• Manage and maintain patient and caregiver schedules
• Coordinate care assignments based on caregiver compatibility and patient needs
• Handle restaffing for callouts or changes in caregiver availability
• Confirm completed visits in HHA Exchange or similar systems
• Assist with billing and payroll preparation by reviewing completed shifts and visit logs
• Maintain consistent communication with caregivers and patients, ensuring all needs are addressed in a timely and respectful manner
• Update and track patient and caregiver information accurately within internal systems
• Support the overall care experience by embodying our caregiver-first, patient-centered culture

What We’re Looking For:

• Experience in home care, healthcare scheduling, or a related field strongly preferred
• Familiarity with EVV systems (e.g., HHA Exchange) a plus
• Ability to juggle multiple tasks while staying organized and solution-focused
• Strong communication skills and a calm, respectful demeanor
• Team player mentality with a high level of reliability and professionalism
• Passion for helping people — both caregivers and clients alike
