The Case Manager II/Trainee- COPES provides clinical case management services to adults receiving long-term term services and supports through an Aging and Long-Term Care of Eastern Washington (ALTCEW) program. Evaluate client strengths and needs, complete a comprehensive assessment, determine level of functioning, develop and implement service plans that provide a reliable and safe plan of care that enables clients to achieve a maximum level of health and independence and targets case stabilization, ensuring independence. Complete administrative duties and meet required program deadlines. Coordinate with and perform advocacy with relevant parties and community agencies.
ESSENTIAL DUTIES AND RESPONSIBILITIES includes but is not limited to the following:
  • Adhere to program standards and guidelines and follow case management program protocols.
  • Provide long-term care services responsive to the needs of elderly and adults with disabilities who are eligible for Title XIX Program services.
  • Conduct face-to-face comprehensive assessments at the client's residence to determine level of care, including determining nursing facility level of care needs.
  • Develop ongoing care plans which provide the support necessary to allow clients to live independently.
  • Facilitate the coordination of in-home caregivers (Individual Providers and Agency Providers) for client care needs.
  • Coordinate with ALTCEW and provide advocacy, coordination, consultation and support to clients, providers and stakeholders across multiple disciplines and systems.
  • Perform program charting requirements, correspondence, and other paperwork accurately and in accordance to program deadlines.
  • Authorize service payment through Provider One/Individual Provider One.
  • Conduct interdisciplinary case reviews and consultations.
  • Establish and maintain a good working relationship with FBH staff and community partners. Perform other related duties as required.
  • Comply with all State WAC requirements and FBH policies and procedures.
  • Requires travel to see clients in the community and travel between work sites.
  • Other duties as assigned.

QUALIFIED CANDIDATES WILL HAVE:
REQUIRED:
  • High School diploma or its equivalent and one year paid full time on-the-job social service experience or one year full-time equivalent verifiable hours through an internship or volunteer work in social services. Part-time hours may be considered if hours total one year full-time equivalent.
  • Must apply for a Counselor Agency Affiliated - Registered credential from the WA State Department of Health within 30 days of hire.
  • Must have an unrestricted and valid driver's license, active car insurance and must be willing to use their personal vehicle in the course of work

PREFERRED:
  • Master's degree in behavioral or health sciences and one year paid on-the-job social service experience; OR
  • Bachelor's degree in behavioral or health sciencesand two years of paid on-the-job social service experience; OR
  • Master's degree in any other field, and three years of paid on-the-job social service experience. OR
  • Bachelor's degree in any other field and four years of paid on-the-job social service experience.

Applicants who do not meet the PREFERRED qualifications will be hired as a "Case Manager Trainee". Case Manager Trainees will no longer be considered a Trainee when all conditions below have been satisfied regardless of whether their length of employment as a Case Manager Trainee would qualify them under the preferred qualifications to be a Case Manager II.
  • Participate in a two-year, on-the-job case manager training program under direct supervision;
  • Participate in a monthly supervisory review of a sample of client assessments and service plans;
  • Participate in annual performance evaluations conducted by the supervisor; and
  • Perform competently as a case manager at the end of two years as determined by the supervisor's performance evaluation.
  • Agency has the discretion to reduce the timeframe to one year for exceptional performance by the Case Manager Trainee as determined by the supervisor's performance evaluation.

KNOWLEDGE, SKILLS & ABILITIES:
  • Ability to communicate and engage effectively with individuals of varied backgrounds, cultures, education level and socio-economic status.
  • Knowledge of elder care and disabled adult caregiving issues.
  • Ability to perform face-to-face assessments and screenings.
  • Knowledge of data entry and computer systems to complete tasks.
  • Ability to complete administrative tasks and organize and prioritize tasks.

  • This position provided client services in-office and within the community at large on a occasional to frequent basis. Community settings include but are not limited to residences, group/congregate living settings, hospitals, shelters, the streets or anywhere in the community at large.

  • Occasional frequent travel between locations is required.
  • The ability to drive a vehicle, traverse distances up to several city blocks and on various terrains, navigate stairs, get in and out of vehicles frequently is required.
  • Daily use of a computer is required.
  • May be required to lift 10 - 20 pounds.
  • Blood-borne Pathogen Category III - No predictable exposure to blood-borne pathogens.

PAY RANGE (Hourly/ Non-Exempt):
  • Case Manager Trainee - (Does NOT meet PREFERRED qualifications as identified above) - $24.30 - $29.60.
  • Case Manager II (Meets PREFERRED qualifications as identified above) - $25.89- $31.53.

All pay is based on longevity at Frontier Behavioral Health; thus the starting pay is as follows:
  • Case Manager Trainee - (Does NOT meet PREFERRED qualifications as identified above) - $24.30 .
  • Case Manager II (Meets PREFERRED qualifications as identified above) - $25.89.
