Licensed Practical Nurse (LPN)
Job Type: Full Time
Shift: Nights
36 hour weeks (12 hour shift 7 - 7:30)

How you'll contribute
A Licensed Practical provides direct patient care to patients using the nursing process in accordance with applicable scope and standards of practice.
Essential Functions
  • Demonstrates knowledge of medications and their correct administration based on the age and clinical condition of the patient
  • Performs patient care considering needs specific to the standard of care for patient's age; includes geriatric and general inpatient rehabilitation patients
  • Under the supervision of the RN, assists with the assessment on all patients and reassessments as per policy. Identifies and initiates appropriate rehabilitation nursing interventions
  • Under the supervision of the RN, performs timely and accurate QI assessments
  • Carry out the plan of care as indicated by the patient's needs and response to treatment; evaluates overall plan daily for effectiveness; updates interdisciplinary care plan accordingly based on changes in patient's condition and individual needs
  • Interacts professionally with patient/family and involves patient/family in the formation of the plan of care
  • Demonstrates ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management
  • Other duties as assigned

  • Strong interpersonal and documentation skills required.
  • Familiarity with rehab documentation systems is preferred.

What we're looking for
Applicants should have a Licensed Practical Nurse license in the state of Indiana. Additional requirements include:
  • Graduation from an accredited Bachelor of Science in Nursing, Associate Degree in Nursing or Nursing Diploma program.
  • Current state licensure as Licensed Practical Nurse.
  • CPR/BCLS certification. ACLS preferred.
  • Excellent oral and written communication and interpersonal skills.
