Location: White Blossom Care Center - 1990 Fruitdale Ave, San Jose, CA 95128

Salary Range: $135,000 - $150,000 / year (Depending on Experience)

We are a premier 153-bed skilled nursing and short-term rehabilitation facility proudly featuring the largest rehabilitation gym in the Bay Area. Operating as a dedicated in-house therapy team rather than a contract agency, we offer unmatched clinical resource space, cutting-edge modalities (including E-Stim, Ultrasound, and Diathermy), and a supportive environment built around functional recovery.

Position Overview

We are seeking a motivated and compassionate clinical leader to step into the dual role of Assistant Director of Rehab (ADOR) and Occupational Therapist (OTR). In this role, you will split your time between delivering exceptional, outcomes-driven patient care and assisting the Director of Rehab (DOR) in managing daily gym operations, leading team meetings, tracking compliance metrics, and mentoring clinical staff. This is an ideal position for an experienced OTR looking to transition into formal clinical leadership within a high-volume, resource-rich facility.

Key Responsibilities

  • Clinical Leadership: Partner with the Director of Rehab to supervise, schedule, and support a massive, energetic team of therapists and COTAs.

  • Operational Oversight: Assist with utilization reviews, daily department auditing, clinical compliance monitoring, and care conferences.

  • Evaluations & Treatments: Perform comprehensive evaluations and deliver evidence-based occupational therapy interventions.

  • Caseload Management: Oversee a diverse clinical caseload including Orthopedic, Neurological, Cardiac, and Vascular rehabilitation.

  • Documentation Excellence: Ensure timely and precise completion of evaluations, daily notes, and recertifications across the department.

  • Discharge Planning: Coordinate durable medical equipment (DME) ordering and provide family/caregiver training to ensure safe discharges.

Qualifications & Requirements

  • Education: Graduate of an accredited Occupational Therapy program.

  • Licensure: Active and unencumbered Occupational Therapist (OTR) license issued by the California Board of Occupational Therapy.

  • Experience: Minimum of 2-3 years of clinical experience in a skilled nursing facility (SNF) or post-acute care setting preferred. Previous leadership or mentoring experience is highly valued.

  • Skills: Strong understanding of Medicare PDPM guidelines, clinical documentation software, and team coordination.
