Occupational Therapist - Rehab Program Coordinator
Innovate Rehab and Wellness
Innovate Rehab and Wellness, in partnership with Hillcrest Health & Living, is seeking a Rehab Program Coordinator to lead and elevate therapy services within our senior care community partner in Iowa.
This is an opportunity to step into a leadership role where you'll shape clinical outcomes, mentor a multidisciplinary team, and drive excellence in rehabilitation services.
What You'll Do
As the Rehab Program Coordinator, you'll serve as the clinical and operational leader of the therapy team, ensuring high-quality, efficient, and compliant care.
Key Responsibilities:
  • Lead and coordinate an interdisciplinary therapy team (PT, OT, SLP) to deliver exceptional patient outcomes
  • Oversee scheduling, caseload management, and team productivity
  • Manage and monitor the PPS process, ensuring accurate coding and care planning
  • Ensure compliance with federal/state regulations and therapy practice standards
  • Drive quality improvement through QAPI initiatives and performance monitoring
  • Collaborate with nursing and facility leadership on long-term care programming
  • Monitor therapy plans of care, interventions, outcomes, and fiscal performance
  • Participate in triple check processes and annual budget planning
  • Support onboarding, mentoring, and development of therapy team members
  • Provide direct patient care within your licensed discipline as needed
  • Ensure equipment, supplies, and therapy environments meet safety and operational standards

What We're Looking For
Education:
  • Graduate of an accredited Occupational Therapy program

Experience:
  • Minimum 3 years of therapy experience in acute, post-acute, or skilled nursing settings
  • Prior leadership or operational management experience preferred

Licensure:
  • Active OT license in Nebraska or Iowa (based on location)

Additional Requirements
  • Ability to manage multiple priorities in a fast-paced environment
  • Strong communication and leadership skills
  • Comfortable working with diverse patient populations and emotional situations
  • Ability to lift up to 35 lbs and perform physical job tasks
  • Proficiency with documentation systems and healthcare technology
