Klarus Home Health is seeking a Registered Nurse to be our Director of Clinical Services to join our team. The Director of Clinical Services is a registered nurse who provides oversight of all patient care services and personnel within our Albuquerque, NM and surrounding territories.


The Home Health Director of Clinical Services (DCS) is responsible for overseeing the clinical operations of the Home Health agency, ensuring high-quality patient care, regulatory compliance, and effective interdisciplinary coordination. This role provides leadership, direction, and mentorship to clinical staff while supporting organizational goals, operational efficiency, and patient outcomes.

Requirements

  • Provide oversight and leadership of all home health clinical operations.
  • Supervise RN Case Managers, LPNs/LVNs, CNAs, therapists, and other clinical personnel.
  • Ensure compliance with Medicare, state regulations, accreditation standards, and agency policies.
  • Review and approve care plans, OASIS documentation, and clinical workflows.
  • Lead Quality Assurance & Performance Improvement (QAPI) initiatives.
  • Oversee patient care delivery to ensure clinical excellence and patient satisfaction.
  • Coordinate interdisciplinary case conferences and ensure timely communication among providers.
  • Manage staffing levels, productivity, and caseload distribution.
  • Collaborate with leadership to develop and monitor budgets and clinical KPIs.
  • Participate in onboarding, training, and competency validation of clinical staff.
  • Support performance evaluations, corrective actions, and professional development initiatives.

Qualifications:

  • Active Registered Nurse (RN) license in the state of practice (or compact license).
  • Minimum 2-3 years of home health clinical leadership experience.
  • Thorough understanding of Medicare Conditions of Participation and OASIS requirements.
