Key Responsibilities
  • Clinical Care: Administer medications to residents safely and accurately in accordance with their specific care plans.
  • Physician Orders: Perform treatments as assigned per physician's orders, staying strictly within the LPN scope of practice.
  • Resident Monitoring: Carefully observe, report, and record resident conditions. Identify signs and symptoms indicating a change in status and communicate them to the RN or Unit Manager.
  • Documentation: Maintain precise and timely electronic medical records (EHR) to ensure continuity of care across all shifts.
  • Regulatory Compliance: Perform all duties in accordance with local, state, and federal guidelines, as well as facility policies.
  • Collaborative Duties: Support nursing leadership with additional tasks as assigned to ensure high standards of facility operation.

Qualifications
  • Education & Licensure: Must hold an active Licensed Practical Nurse (LPN) license eligible for practice in the state of Wisconsin.
  • Technical Skills: Proficiency with computers and Electronic Health Records (EHR) is required.
  • Professionalism: Ability to work effectively as part of a multidisciplinary team or independently when required.
  • Experience: Previous experience in a long-term care (LTC) setting is preferred, though we welcome all qualified applicants with a heart for senior care.
