Position Description

The Medical Coordinator works together with other Medical Coordinators, Registered Nurse, and Medical Systems Supervisor to provide effective delivery of medical services to all residents of the Northwest Passage programs.

This is a part-time position, approximately one day per week, with additional opportunities to provide coverage as needed.

Essential Duties and Responsibilities include the following. Other duties may be assigned.

  • Provide nursing and/or medical coordination services at assigned sites.
  • Establish and maintain effective working relationships with healthcare providers involved in the care of residential clients.
  • Administer and document prescribed medications in accordance with prescribing provider orders and agency policies and procedures.
  • Collect, document, and monitor medical information and concerns during intake and throughout treatment.
  • Request, track, and obtain necessary medical records.
  • Coordinate and schedule medical appointments, including acute care visits, laboratory services, intake appointments, and follow-up care.
  • Ensure clients remain current on preventative healthcare needs, including well-child examinations, dental care, and vision screenings.
  • Prepare and submit required documentation to clinics, pharmacies, and other healthcare providers.
  • Communicate client observations and relevant health information to prescribing clinicians, including medication effectiveness, side effects, PRN medication use, and overall functioning.
  • Relay recommendations and updates from prescribing clinicians to treatment and residential staff.
  • Order, refill, organize, and package medications as needed.
  • Maintain supplies of over-the-counter medications and off-site medication kits.
  • Participate in case management, treatment team, and other interdisciplinary meetings as needed.
  • Conduct and document routine health assessments, including vital signs, height, weight, and medication side effect monitoring.
  • Provide formal and informal education to staff regarding medical conditions, medications, and client-specific health needs.
  • Complete medical discharge summaries and related documentation.

Qualifications

  • Current Wisconsin license as a Licensed Practical Nurse or certification as a Medical Assistant.
  • Demonstration of knowledge of and ability to follow professional standards of practice in the fulfillment of all job duties
  • Ability to use professional experience and well-developed reasoning skills to make professional judgments.
  • Ability to learn and adapt.
  • Ability to work independently and within a team. Ability to be self-directed.
  • Ability to maintain various forms of detailed documentation.
