Position Overview

RN Case Manager Signature Healthcare at Home, part of The Pennant Group, is seeking a fulltime, nonexempt Registered Nurse Case Manager to support our westside Portland service area. This role is ideal for nurses who value autonomy, meaningful patient relationships, and the opportunity to deliver highquality care directly in patients' homes.

As an RN Case Manager, you will manage a balanced caseload, provide skilled nursing interventions, coordinate interdisciplinary services, and guide patients and families through recovery, chroniccare management, and safe transitions.

Key Responsibilities

RN Responsibilities

  • Complete comprehensive nursing assessments and develop individualized care plans.

  • Provide skilled nursing interventions in patient homes, including wound care, medication management, chroniccondition support, and patient education.

  • Manage a fieldbased caseload with strong organizational efficiency.

  • Coordinate care with interdisciplinary team members to ensure seamless service delivery.

  • Communicate effectively with physicians, caregivers, and internal team members.

  • Maintain accurate, timely documentation in accordance with regulatory and organizational standards.

  • Support safe transitions of care from hospitals, SNFs, and outpatient settings.

Qualifications

RN Requirements

  • Current Oregon RN license (or ability to obtain).

  • One year of nursing experience preferred; homehealth experience a plus.

  • Strong assessment, communication, and criticalthinking skills.

  • Ability to work independently in a fieldbased role.

  • Reliable transportation and valid driver's license.
