SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
* Provide direct services to people in the areas of housing, risk mitigation, transportation, skill building, money management, medication administration and other treatment based plans.
* Work closely with families and early intervention provider staff, to ensure proper services and supports are delivered according to early intervention best practices.
* Collaborate with the patient and patient's family or significant others to implement an effective service plan, explaining the available clinical options to the team, including the advantages and disadvantages of each option.
* Communicate and document patient's progress toward their recovery.
CLIENT'S REQUIRED SKILLS AND EXPERIENCE
* Associate's or bachelor's degree in a related field and/ or equivalent to two years' relevant human services experience with population that has persistent mental illness and/or substance use disorder.
* Bi-lingual fluency in Spanish is preferred not required; lived experiences are welcomed and valued
* Ability to complete and pass Criminal Background Check
