Job description

We are seeking an experienced Registered Nurse Case Manager to join a collaborative healthcare team. In this role, you will coordinate patient care, facilitate discharge planning, connect patients and families with appropriate resources, and work closely with interdisciplinary teams to ensure high-quality patient outcomes throughout the continuum of care.

Schedule

  • Day Shift
  • 8:00 AM – 4:30 PM
  • Rotating Weekends

Compensation & Incentives

  • Salary Range: $34.25 – $47.10/hour
  • Sign-On Bonus: Case by Case Basis (Not Guaranteed)
  • Relocation Assistance: Case by Case Basis

Key Responsibilities

  • Guide patients and families through program orientation, discharge planning, and available resources.
  • Explain Medicare, insurance benefits, patient rights, and discharge criteria.
  • Coordinate patient, family, and caregiver education to support treatment planning and recovery goals.
  • Develop and implement individualized care and discharge plans.
  • Complete psychosocial assessments and discharge planning evaluations.
  • Collaborate with physicians, nursing staff, and interdisciplinary teams to ensure safe patient transitions.

Qualifications

  • Active Registered Nurse (RN) License required.
  • Associate Degree in Nursing or Nursing Certificate required.
  • Minimum 2 years of recent Acute Care experience within the last 3 years required.
  • Experience in one or more of the following preferred:
    • ICU
    • CCU
    • Med-Surg
    • LTAC
    • Hospital Case Management
  • Strong critical thinking, communication, and organizational skills.
  • Ability to collaborate effectively with physicians and multidisciplinary teams.
  • Comfortable advocating for patients and participating in complex discharge planning discussions.

This is an excellent opportunity for an experienced Acute Care RN looking to transition into or continue growing within Hospital Case Management while working in a patient-focused and supportive healthcare environment.
