Job Summary:  The Oaks of O'Fallon Senior Living 

The Wellness Director (LPN/RN) is responsible for ensuring the delivery of high-quality resident care and clinical services in compliance with Missouri Residential Care Facility (RCF) regulations. This position provides leadership to the caregiving team, oversees clinical programs, maintains regulatory standards, and collaborates with the Executive Director to achieve organizational goals related to resident wellness and satisfaction.

Key Responsibilities:
  • Resident Care & Clinical Oversight
    • Oversee development and monitoring of personalized care plans.
    • Conduct resident assessments and ensure accurate clinical documentation.
    • Supervise medication management processes, including administration and storage.
  • Regulatory Compliance
    • Maintain current knowledge of Missouri RCF rules and ensure ongoing compliance.
    • Conduct internal audits and ensure readiness for inspections or surveys.
    • Implement corrective action plans when needed.
  • Team Leadership
    • Hire, train, and supervise care staff, nurses, and med techs.
    • Provide coaching and performance management to build a strong, engaged care team.
    • Coordinate staffing schedules to meet resident care needs.
  • Quality & Safety
    • Monitor resident outcomes and clinical indicators for quality assurance.
    • Lead risk management efforts and incident investigations.
    • Drive continuous improvement through data-driven action plans.
  • Collaboration & Communication
    • Partner with the Executive Director and interdisciplinary team to deliver excellent service.
    • Build trusted relationships with residents, families, and healthcare providers.
    • Represent Cornerstone’s values and communicate across all levels of the organization.
  • Administrative Responsibilities
    • Manage the clinical budget and ensure efficient use of resources.
    • Maintain organized and compliant clinical documentation and resident files.
    • Participate in leadership meetings and contribute clinical insights to operations.

Qualifications & Requirements:

  • Current, unencumbered LPN/RN license in the State of Missouri.
  • Minimum of 2–3 years of clinical experience in senior living, long-term care, or related healthcare setting
  • Previous leadership or supervisory experience required; experience in a Residential Care Facility or Assisted Living facility strongly preferred.
  • Thorough understanding of Missouri RCF regulations, including medication management and resident rights.
  • Strong leadership, problem-solving, and organizational skills.

  • Proficient in care planning, clinical assessment, and documentation procedures.

  • Excellent communication skills and ability to work collaboratively with diverse groups.
  • Ability to work flexible hours as needed, including weekends or evenings for on-call coverage.

  • Must pass background checks and any other pre-employment screening as required by state regulations and company policy
