Triage, Consultation, and Wellness Response Team (TCWRT)
Mental Health and Wellness (MHW) Program โ€” AB 109 Reentry Services

Program Description
The Triage, Consultation, and Wellness Response Team (TCWRT) is a Felton Institute program funded by the Alameda County Probation Department (ACPD) and the Alameda County Behavioral Health Department (ACBHD) under the AB 109 Mental Health and Wellness (MHW) initiative. TCWRT serves AB 109-eligible clients โ€” including individuals released from Santa Rita Jail or CDCR facilities, and those on Post-Release Community Supervision, formal probation, or in specialty court programs โ€” by connecting them to mental health, substance use, and wellness services. The team provides clinical consultation and triage, peer and supportive services, wellness response and de-escalation, outreach and mental health promotion, and administration of the Specialty Provider Pool (SPP). TCWRT staff are stationed across the Oakland and Hayward ACPD offices, the Center of Reentry Excellence (CORE), and AB 109 Housing Program locations, and the program operates under a detailed set of contractual reporting, data, and fiscal compliance requirements.

Position Summary
This position blends clinical supervision of TCWRT case management and peer support staff with the programโ€™s quality assurance, data, and contractual reporting functions, ensuring both the clinical integrity and the contractual compliance of services delivered under the AB 109 Mental Health and Wellness contract. This position reports to the Division Director/Medical Director.

Clinical Supervision and Case Consultation

  • Provide clinical supervision of TCWRT case management and peer support staff, and co-sign mental health assessments and service documentation completed by unlicensed staff.
  • Provide clinical consultation to TCWRT staff and when appropriate, Deputy Probation Officers on client mental health status, risk, and appropriate level of care, incorporating Motivational Interviewing, Harm Reduction, Trauma-Informed Care, and Recovery-and-Wellness principles.
  • Review client mental health assessments, Provider Service Plans (PSPs), and closing summaries prior to submission, ensuring clinical accuracy and adherence to County and Felton documentation standards.
  • Contribute to case conferences and team meetings, presenting clinical considerations and facilitating exchange of information among TCWRT staff.
  • Support staff in applying evidence-based practices, including Motivational Interviewing, Cognitive Behavioral Therapy, Thinking for Change, and Wellness Response de-escalation techniques.
  • Maintain client confidentiality and adhere to all applicable Federal, State, and local laws and mandated reporting requirements.

Quality Assurance, Data, and Contractual Reporting

  • Work with Program Manager & Analytics Team to maintain the programโ€™s data infrastructure, including trackers and dashboards, to capture all data elements required by the AB 109 Scope of Work and the ACPD Provider Portal.
  • Work with administrative staff to conduct routine chart audits and documentation reviews to verify timeliness, accuracy, and completeness of clinical and administrative records, and provide feedback to case management and peer support staff.
  • Assist in monitoring program performance against all AB 109 process, quality, and impact objectives, including outreach, enrollment, triage, and Wellness Response timeliness targets.
  • Identify performance trends and documentation gaps and support development and implementation of corrective action and continuous quality improvement plans.
  • Maintain a master log of client Releases of Information (ROIs) and adverse event reports and ensure timely reporting per contractual requirements.

Program and Staff Support

  • Support training of TCWRT clinical and peer support staff on documentation standards and evidence-based practices.
  • Track staff licensure, certification, and training completion, and maintain records of workforce composition requirements.
  • Support administrative staff in maintaining consistent communication with ACPD and ACBHD on clinical, data, and compliance matters.
  • Other duties as assigned.

Minimum Qualifications:

  • Masterโ€™s degree in Social Work, Psychology, Counseling, or a related clinical field, with current licensure as an LCSW, LMFT, or LPCC (or license-eligible with active clinical supervision, per program requirements).
  • Minimum of two (2) years of clinical case management or supervisory experience with adult populations with serious mental illness, substance use disorders, and/or corrections involvement.
  • Minimum of one (1) year of experience in program data management, quality assurance, or contract compliance reporting, preferably within a behavioral health or justice-involved services setting.
  • Working knowledge of Motivational Interviewing, Harm Reduction, Trauma-Informed Care, Cognitive Behavioral Therapy, and Recovery-and-Wellness principles.
  • Working knowledge of Microsoft Word, Excel, and PowerPoint, and experience with electronic health records or provider portal systems.
  • Knowledge of confidentiality and reporting requirements under HIPAA, 42 CFR Part 2, and applicable California law.
  • Strong organizational, interpersonal, and communication skills, with the ability to prioritize and manage multiple clinical and compliance deadlines.
  • Ability to work cooperatively on a multidisciplinary team with clients and colleagues of diverse ethnic and cultural backgrounds.
