Responsibilities
  • Monitor and evaluate through data collection that can include interviews on Nursing performance and delivery of patient care. Conducting audits and reviews completed in a period that allows for timely analysis of areas needing improvement specific to regulations and standards of nursing care.
  • Analyze resident care data through clinical documentation, external Quality Measures, and concerns from staff and/or residents to identify trends. Conduct comprehensive audits on medical records to assess accuracy and adherence to policies, practices, standards of care. Through the analysis of the results, develop in collaboration with Education a teaching plan. Validate compliance with further audits determining if education plan delivered the outcomes desired. Re-evaluate plan and continue the process with problem solving concepts for QAPI.
  • Investigate all self- reports to the State, nursing complaints, and adverse events for compliance to documentation to include the event, documentation prior to event and identification of documentation required as follow up to event. Conduct root cause analysis with all stakeholders implementing corrective actions to include education. Use the 'Plan Check Do Act' concept for QAPI.
  • Work closely with health care teams, not just limited to Nursing, to establish best practices. Collaborate with Education to design and implement staff training targeting areas for improvement. Design and implement monitoring processes that measure the success of the training and identify areas to continue to improve.
  • Assist in the development and review of resident care policies and procedures to align with quality standards. Develop monitoring processes to measure compliance with policy and procedures to identify areas of improvement.
  • Prepare and present reports on quality improvement initiatives, tracking processes and outcomes to the residents.
  • Collaborate with Nursing departments on areas of improvement to monitor action plans to determine compliance. Identify strengths and weaknesses and in collaboration with nursing departments, develop robust action plans with continued emphasis on improvement.
  • Participate in interdisciplinary team meetings and committees to review opportunities for QAPI interventions.
#INDHP Requirements Qualifications
  • Registered Nurse or Licensed Practical Nurse license in the State of Maryland
  • 3 years in Long Term Care/Skilled Nursing Environments
  • Familiarity with healthcare regulations to include the survey processes
  • Strong analytical and problem-solving skills
  • Certification in Quality Assurance or Quality Management (Six Sigma, Lean) a plus
  • Proficient in using Point Click Care and other related online websites
