Job description

Must have a Bachelor's or Master's degree in Human Services field from an accredited college with and active QMHP -C, QMHP-A or QMHP-T certification.

Provide direct In-Home counseling services to identified youth and their families.  Assists in planning services. Provides parent education support.  Provides a variety of interventions in youth's home and community as appropriate and necessary.  

Actively coordinates and plans discharge and aftercare plans with referring agency.

Attends and participates in team meetings. 

Attends and participates in orientation, in-service training, and conferences as scheduled.

Attend supervisions.

Performs other duties as assigned.
