Pay Range: $24.00 - $28.00/hour
Job Summary:
The Quality Improvement (QI) Investigator is responsible for facilitating investigative activities that support the QI department in carrying out agency incident special investigations. Will be responsible for reportable incidents (alleged abuse/neglect and significant incidents) and notable occurrences. This consists of investigation activities including assignments, notifications, and monitoring the investigation process to complete an investigation timely and accurately.
Essential Job Duties:
  • Under the direction of the QI Coordinator, conducts timely and thorough Special Investigations, including managing specific incident situations, including notifications, direct communications with impacted individuals, documentation, data entry, and completion of all related record keeping; interviewing witnesses within the required timeframe; collecting, documenting, and preserving evidence in line with regulatory requirements; writing; submitting the investigation report to the Director of QI for review/approval; and final submission of the report based on agency policy.
  • Facilitates and acts as a Liaison for Justice Center and other State Oversight Agencies: schedule witnesses, gather documentation, etc.
  • Reviews minor incident reports, ensures proper documentation and consistent accurate information and thorough data entry is submitted to various internal and external data collection systems (NIMRS, IRMA, etc.).
  • Ensures compliance with rules and regulations pertaining to incident management and other QI activities.
  • Attends all required internal training to maintain competence in position and attends external trainings as directed or per request, for ongoing professional growth and development.
  • Complies with all federal and state regulations and agency policies pertaining to Health Insurance Portability and Accountability Act (HIPAA), false claims, whistleblower policies, and other applicable federal and state laws and regulations.
  • Maintains confidentiality regarding privileged administrative and client information.
  • Attends and participates in staff and other meetings as assigned.
  • Performs all other duties as assigned by Supervisor.

Minimum Education Required:
  • Associate's Degree in Human Services or related field

Minimum Experience Required:
  • Three (3) years in human services or education field

License/Registration/ Certifications Required:
  • New York Driver's license

Training Requirements:
  • New Hire Orientation

Physical Requirements:
  • The overall nature of the position is sedentary requiring little physical effort with occasional light physical exertion required.
  • There is little, if any, exposure to environmental conditions. The constant physical demand of the position is sitting.
  • The frequent physical demands of the position are standing, walking, climbing, talking, hearing, repetitive motions, and eye/hand/foot coordination.
