Home of the Innocents is seeking a Wound Care Nurse to lead and coordinate the Skin and Wound Management Program at the Kosair for Kids Complex Care Center, our pediatric skilled nursing facility. In this specialized role, you’ll drive evidence-based wound prevention and treatment, serve as the clinical subject matter expert, and collaborate across disciplines to protect the health and quality of life of medically complex children and young adults.

A Day in the Life

The Wound Care Nurse leads all aspects of our Skin and Wound Management Program as an integral component of the Infection Prevention and Control Program. Reporting to the Director of Clinical Compliance, you’ll collaborate with the QAPI Manager, Infection Prevention Nurse, Nursing leadership, physicians, and the interdisciplinary care team to ensure wound practices are evidence-based, compliant, and outcome-driven.

Schedule: Full-time, non-exempt working primarily Monday–Friday, 8:00 AM – 5:00 PM, with occasional evening, weekend, or holiday hours to support departmental needs.

How This Position Contributes to Our Mission

• Lead all aspects of skin and wound management including assessment, prevention, treatment, and documentation for residents
• Serve as the SME for wound care; proactively identify high-risk residents, implement mitigation strategies, and lead surveillance activities
• Collect, track, and analyze wound data; prepare reports and present findings to the QAPI Committee and other stakeholders
• Evaluate wound protocols for regulatory compliance; respond to deficiencies, support inspections, and assist with plans of correction
• Provide education, guidance, and retraining to staff on wound prevention, evidence-based practices, and regulatory standards
• Collaborate with the Quality Improvement and Compliance Department to align wound management practices with federal, state, and facility standards

Skills and Qualifications

Required:

• Associate’s Degree in Nursing; unencumbered RN license in Kentucky. BSN preferred
• Minimum two (2) years of RN experience in acute or long-term care, experience with developmental and intellectual disabilities preferred
• Certified Wound Care Nurse (CWCN), Certified Wound Ostomy Nurse (CWON), or CWOCN credential, or willingness to obtain upon hire
• Strong knowledge of long-term care standards and wound management regulatory requirements
• Current CPR/BLS certification (provided if needed)
