Our Clinical Supervisors are responsible for ensuring the care and services are delivered appropriately as well as supervise clinical personnel.

On An Average Day You Will:
(Includes, but not limited to)

  • Receive case referrals and review available patient information related to the case, including home visits, to determine hospice/home care needs
  • Assign appropriate hospice personnel to referral case as needed
  • Confer with Medical Director and Attending Physician regarding any questions as to an individual's eligibility for services
  • Review and evaluate each case through a variety of means such as home visits, conferences, records review, and the services provided by clinicians
  • Review patient's medical diagnosis, prognosis, medications, procedures and clinical course
  • Discuss and verify impressions, instruct and guide clinicians to promote more effective performance and delivery of quality home care services
  • Available, during operating hours, to assist clinicians as appropriate
  • Conduct clinical record reviews and communicate findings and recommendations to the Clinical Director and hospice personnel, as defined by the QAPI Program
  • Assist in the screening and interviewing process of new employees and make recommendations for employment of individuals
  • Assist in the orientation of new employees, as needed
  • Assist the Director of Patient Care Services and other supervisory hospice personnel in the planning, implementation and evaluation of in-service and continuing education programs
  • Assist in the formulating, revising, implementing and evaluating organization policies, procedures, goals and objectives - both short- and long-range
  • Comply with accepted professional standards and principles
  • Assess the health needs of the community and assist in the development of plan(s)/program(s) to meet needs
  • Participate in public relation and community activities that promote the organization's role as an effective member of the hospice and health care delivery system
  • Promote customer service orientation to all hospice personnel
  • Participate actively in quality assessment performance improvement teams and activities as directed by the Director of Patient Care Services in accordance with the QAPI Program
  • Perform other duties and activities as delegated by the Director of Patient Care Service
  • Additional duties as assigned


Qualifications:

  • A Registered Nurse license in good standing to practice professional nursing in the state
  • Must have previous experience in a hospice home care setting with two years of management or supervisory experience
  • An enthusiasm for providing the best possible care to patients and their families
  • Understanding of the hospice philosophy and issues of death and dying
  • Excellent observation, verbal, and written communication skills
  • A valid driver's license
  • Current CPR certification
