Job description

Care Service Coordinator /Home Care Scheduler- $2,500 Sign-On Bonus*

About Us
At Caring For Life, we believe in weaving together compassion and efficiency to provide the absolute best home care for our clients across Northern Virginia. We don’t just fill shifts; we build meaningful connections.
We are looking for a dedicated Care Service Coordinator -Full Time Onsite to join our team. We don’t need a passive order-taker; we need a strategic puzzle-master who sees scheduling as a dynamic challenge that requires empathy, quick thinking, and a proactive mindset. If you thrive when thinking on your feet and love finding creative solutions to fast-moving logistics, you will love it here.
"Thinking Outside the Box"
In home care, "Plan A" can change in a heartbeat. We need a coordinator who doesn't panic when a wrench gets thrown in the gears, but instead looks at the big picture from multiple angles:
  • The Puzzle Master: You don’t just look at an open shift and blast out generic messages. You analyze your care team’s strengths, availability, and personality styles to creatively bridge gaps and make the perfect client match.
  • Proactive Pivoting: When a scheduling disruption happens, you don’t wait around for instructions. You anticipate the logistical ripple effects, look for alternative solutions, and present answers rather than just flagging problems.
  • Precision Communication: You understand that while warmth is crucial, clarity is king. You know how to pivot from a friendly phone call to generating a precise, formal email trail so the entire team stays perfectly aligned and documented.

Core Responsibilities
  • Schedule Coordination: Masterfully prepare and manage daily, weekly, and weekend schedules for our field staff. You ensure every client receives seamless, high-quality care by thinking three steps ahead of potential gaps.
  • Recruitment, Screening & Onboarding: Identify, screen, and vet Care Partners who genuinely embrace our organization's core values of compassion, care, and dedication.
  • The Vital Communication Link: Serve as the central nervous system of our daily operations. Accurately and promptly communicate scheduling changes between the office, caregivers, and clients with a warm, professional, and reassuring approach.
  • Meticulous Record Maintenance: Keep flawless digital logs, schedules, and profile updates for both clients and care partners. Your strict attention to detail ensures compliance, smooth operations, and a high standard of care.
  • On-Call Rotation: Participate in a weekly after-hours on-call rotation. This is where your problem-solving skills truly shine—responding calmly, empathetically, and efficiently to last-minute emergency calls to ensure our clients and care partners are always supported.
  • Team Collaboration: Partner closely with other management team and field staff to understand their unique needs and preferences, fostering a harmonious, supportive work environment.  Willing to listen to feedback, suggestions, recommendations and make it your own without thinking that you are being micromanaged.

  • Transparency & Accountability: Provide honest communication when you need assistance or would like to move in a different direction in coordinating for your client.  Ensure that all your work and follow up is communicated clearly and effectively with the care team.  Take accountability for mistakes without making needless & time-wasting excuses.

Skills & Qualifications
  • Experience Required: Minimum of 1 year of experience as a Home Care Scheduler or in a similar fast-paced coordination role (Home Care, Hospice, or Home Healthcare highly preferred).
  • High Operational IQ: Strong organizational skills with an elite eye for detail—especially when it comes to emails, texts, and official communications.
  • Tech-Savvy Adaptability: Comfortable and experienced navigating Microsoft Office and scheduling software (direct experience with systems like AxisCare or WellSky is a huge plus).
  • Balanced Communication: Exceptional interpersonal skills. Ability to communicate on the phone and via text in a warm, empathetic manner, while maintaining professional rigor in writing.
  • The Logistic Chessmaster: A proactive, resourceful approach to tasks & operations. When a sudden schedule disruption happens, you don't panic or wait for instructions.
  • Education: High School Diploma or GED required; College degree preferred.
  • Preferred Credentials: Bilingual in Spanish is highly encouraged and preferred.

Benefits & Perks
  • Competitive Compensation & Bonuses
  • Comprehensive On-the-Job Training
  • 401(K) Retirement Plan
  • Paid Time Off
  • Sign On Bonus $2500 ($500 at 30 days, $1,000 at 90 days, and $1000 at 180 days)*
