Registered Nurse (RN) โ€“ Home Health

Albuquerque & Rio Rancho, New Mexico

We are seeking an experienced and compassionate Registered Nurse (RN) to join our growing Home Health team serving patients throughout Albuquerque and Rio Rancho.

This position is ideal for an RN who has a strong background in home health, is comfortable working independently in the field, and has handsโ€‘on experience completing Start of Care (SOC) assessments and OASIS documentation.

Minimum Qualifications

Candidates must meet the following experience requirements:

  • Active New Mexico Registered Nurse (RN) license in good standing

  • Minimum 2 years of total nursing experience

  • Minimum 1 year of Home Health nursing experience

  • Minimum 1 year of Start of Care (SOC) and OASIS experience

  • Strong clinical assessment and documentation skills

  • Ability to manage patient care independently in a home setting

  • Excellent communication, organization, and time-management skills

  • Reliable transportation and ability to travel throughout the Albuquerque/Rio Rancho service area

Responsibilities

As a Home Health RN, you will help patients receive highโ€‘quality clinical care while remaining safely and comfortably in their homes. Responsibilities may include:

  • Complete comprehensive Start of Care (SOC) assessments

  • Complete accurate and timely OASIS documentation

  • Perform skilled nursing assessments and visits

  • Develop and update individualized plans of care

  • Monitor changes in patient condition and communicate with physicians and the interdisciplinary care team

  • Provide patient and family education

  • Coordinate care with therapists, physicians, caregivers, and other healthcare professionals

  • Maintain accurate, timely, and compliant clinical documentation

  • Help identify changes in condition and opportunities to improve patient outcomes and prevent unnecessary hospitalization
