Now Hiring: Physical Therapist (PT) - PRN
Make a Meaningful Clinical Impact at Concord Post Acute
Concord Post Acute is seeking a skilled and compassionate Physical Therapist (PT) to join our exceptional rehabilitation team on a PRN/per-diem basis. This is an exciting opportunity for a clinician who values flexibility, professional autonomy, and the ability to make a lasting impact in a collaborative post-acute care environment.
Position Details
  • Position: Physical Therapist (PT) - PRN
  • Location: Concord, CA
  • Compensation: Starting at $64-$65/hour
  • Schedule: PRN / Per-Diem
  • Environment: Supportive, team-oriented, and fast-paced rehab setting

Key Responsibilities
  • Perform comprehensive physical therapy evaluations in a timely manner following physician referral
  • Develop and implement individualized treatment plans focused on mobility, balance, strength, pain management, and functional independence
  • Supervise and collaborate with Physical Therapist Assistants (PTAs) to ensure quality patient care and compliance with state practice standards
  • Participate in interdisciplinary care planning, patient conferences, and utilization review meetings
  • Recommend appropriate Durable Medical Equipment (DME) to support safe mobility and discharge planning
  • Educate residents, families, and caregivers on exercise programs, safety strategies, and long-term maintenance plans
  • Maintain accurate and timely documentation in accordance with regulatory and payer guidelines
