Adult Services:

Case Coordinators are responsible for providing comprehensive case management and support services to individuals with mental health and/or substance use disorders living in the community. This role involves conducting assessments, developing individualized service plans, coordinating care, and advocating for clients' access to community resources. The Case Coordinator will work collaboratively with clients, families, and community partners to promote recovery, independence, and overall well-being.

To support a safe and healthy environment for the individuals we serve and for one another, all employees of Radiant Health are required to comply with organizational health and infection prevention requirements as a condition of employment.
