Physical Therapist Assistant (PTA)
Join Our Team at Rock River Nursing & Rehab in Fort Atkinson, Wisconsin

We are seeking a dedicated and compassionate Physical Therapist Assistant (PTA) to join our team at Rock River Nursing & Rehab in Fort Atkinson, Wisconsin. As a PTA, you will play a vital role in providing high-quality physical therapy services to our patients, promoting independence, and improving overall well-being.

About the Role:

  • Provide physical therapy services to patients under the supervision of a licensed Physical Therapist, including exercises, stretching, and other modalities

  • Assist patients in developing and achieving their individualized treatment plans

  • Document patient progress and communicate with healthcare teams to ensure continuity of care

  • Maintain accurate records and reports of patient treatments and progress

  • Participate in patient education and discharge planning

Responsibilities:

  • Conduct patient evaluations and assessments to establish baseline data and identify areas for improvement

  • Develop and implement treatment plans in collaboration with Physical Therapists and other healthcare professionals

  • Implement and modify exercises, stretches, and other interventions as needed

  • Monitor patient progress and adjust treatment plans accordingly

  • Collaborate with Physical Therapists to ensure continuity of care and effective treatment

Requirements:

  • Associate's degree in Physical Therapist Assistant studies

  • Current certification as a Physical Therapist Assistant (PTA) in the state of Wisconsin

  • 1+ year of experience as a Physical Therapist Assistant in a skilled nursing setting

  • Strong communication and interpersonal skills

  • Ability to lift and transfer patients as necessary

  • Familiarity with electronic medical records (EMRs)
