Job Summary:

Opportunity to pick up additional hours at Monroe Hospital

A ProMedica RN Acute Care Navigator plays a key role in overseeing patient care, seamless care progression, coordination, and the appropriate level of care for their caseload. You focus on optimizing length of stay, preventing readmissions and compliance with value-based programs.

You will collaborate with interdisciplinary teams to conduct initial assessments and develop discharge plans.

This role also requires patient education through providing patients with information on medications, treatments, and post-discharge care. Through the patient's stay, you will monitor their progress, review daily health records, and escalate any issues to leadership to ensure smooth transitions and patient outcomes.

REQUIREMENTS

  • Must have either a BSN, or 5 years' applicable nursing experience
  • A current, active, and unrestricted licensure as a Registered Nurse in the state of practice is required

PREFERRED REQUIREMENTS

  • MSN or higher desired/preferred.
  • Applicable experience in patient advocacy, care management, and knowledge of hospital and community resources
  • Case Management or similar experience
  • License: RN, BSN
  • Certification: Specialty certification in care management (CCM, RN-BC, ACM or similar)
