Overview
Leads, directs and coordinates State of Illinois Program 590-Crisis Care System grant. Completes operational management and implementation of grant requirements, oversees program staff, and oversees and administers the grant budget funds.
Qualifications
Certification & Licensure Requirements
One of the following:
Licensed Clinical Social Worker (LCSW) - Illinois Department of Financial and Professional Regulation (IDFPR);
Licensed Clinical Psychologist - Illinois Department of Financial and Professional Regulation (IDFPR);
Licensed Clinical Professional (LCPC) - Illinois Department of Financial and Professional Regulation (IDFPR);
  • Driver's License - Illinois Secretary of State (ILSOS);
  • Must have control over personal use of all mood-altering chemicals and behaviors. Upon Hire
  • If recovering from an addiction, must currently have a minimum of 2 years of uninterrupted recovery and must actively treat this addiction by appropriate recovery practices including. Upon Hire
  • Use of usual and customary equipment used to perform essential functions of the position. Upon Hire

Responsibilities
MONITORS AND COORDINATES PROGRAM
  • Primary liaison with the funder
  • Maintains staffing patterns and day to day operations of State of Illinois Program 590-Crisis Care System.
  • Ensures compliance with all policies and contractual agreements.
  • Ensures compliance with grant training requirements.
  • Hires for grant positions.
  • Manages cost, schedule, performance and customer satisfaction
  • Provides supervision for data manager.
  • Oversees ERS activities completed by administrative staff.
  • Develops a "no wrong door" system of care with community partners.
  • Develops a centralized data system and standardized intake forms across organizations.
MAINTAINS EFFECTIVE COMMUNCIATION AND TEAMWORK STYLE WITH COWORKERS, OTHER DEPARTMENTS, OTHER HEALTH CARE PROVIDERS AND THE COMMUNITY.
  • Develops, maintains, and facilitates a Planning and Implementation Team.
  • Actively participate in communication, attendance in required meetings, and learning collaborative sessions related to State of Illinois Program 590-Crisis Care grant.
  • Actively maintain professional relationship with open communication with providers, clinics, units, and community partners.
  • Actively maintains open communication & demonstrates a cooperative, teamwork style with patients, families, other staff members, other healthcare workers, medical staff and community.
  • Demonstrates organizational abilities through correct prioritization of duties, efficient and appropriate usage of time, and effective utilization of support personnel.
Required Documentation for Program 590-Crisis Care System grant
  • Ability to understand data and provide analysis of the data.
  • Oversee, guide, and support data manager's collection and reporting of data.
  • Utilizes data to inform executive leadership of process evaluation and improvement in the program.
  • Maintains computer proficiency
  • Responsible for submitting all required reports for Program 590-Crisis Care System grant.
  • Assist with audits
  • Oversees and ensures that data is entered timely and correctly in the identified data base.

Compensation and Benefits
The compensation range for this position is $26.34per hour - $43.99per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model.
