Job description

Position Summary:
Under direct supervision, provides a broad range of paraprofessional social services to youth and their families.
Work Schedule:
The hours for this position are typically 12pm-8pm M, T, W2, TH and 9am-5pm on F. Hours during summer and school breaks are from 8:30am-5pm Monday-Friday. Opportunities for varying hours can be discussed with the program manager.
Job Duties and Responsibilities:
  • Coordinates/provides resiliency support services for all Remix Clubhouse members.
  • Provides skills training groups to members and incorporating evidence-based practices/curriculums.
  • Provides CSI (Community Support Individual) to members on caseload or as needed.
  • Documents to account for all services provided.
  • Provides overall daily care for members, preparing meals, general oversight, etc.
  • Collaborate with parents to coordinate care for members.
  • Works cooperatively with other community agencies which may be serving the member (school system, DFCS, etc.)
  • Charts all contacts with families, community agencies, or other collaborations for the member's care.
  • Advocates for youth with mental health and behavioral challenges.
  • Maintains the strictest confidentiality of all information related to the member's care
  • Assist in coordinating/creating monthly Family Nights
  • Provides transportation for members
  • Monitors members treatment plan goals
  • Works within a team environment and with external stakeholders
  • Attend staff meetings, conferences, workshops, or other meetings as required by the supervisor or agency.
  • Works cooperatively and in a professional manner with all clubhouse staff and other agency personnel.
  • Assists with daily administrative tasks as assigned or needed.
  • Collaborates with community organizations/places of business to incorporate outings into members' treatment.

Minimum Qualifications:
High school diploma or GED AND One year of experience in a social service-related position OR Two years
at an accredited college or university with at least 15 credit hours in a social service-related field.
Preferred Qualifications:
Computer proficiency, previous work with electronic health record systems, and work with children/youth Knowledge of community resources appropriate to the ethnic background of the population served; Knowledge of the ethnic culture, beliefs, customs and health practices common to the ethnic population served.
