Position Overview

The Core For Relationship Healing is seeking an empathetic and skilled Licensed Professional Counselor (LPC) to join our dynamic team. This full-time remote position offers a unique opportunity to provide vital mental health counseling services through our outpatient office, with a salary range of USD 87,000 to 105,000 annually. Key Responsibilities
  • Collaborate with primary care providers to identify and address mental health and psychosocial issues, contributing to a holistic approach to patient care.
  • Conduct thorough patient intakes, focusing on diagnostic and functional evaluations to inform treatment planning and interventions.
  • Provide short-term, solution-focused counseling for patients requiring immediate mental health support, ensuring alignment with established Agency and Federally Qualified Health Center (FQHC) guidelines.
  • Recommend referrals to specialty mental health facilities when necessary, based on clinical assessments and individual patient needs.
  • Engage in proactive follow-up communications to monitor patient progress and adapt treatment plans as necessary.
  • Design and implement relapse prevention strategies, assisting patients in achieving and maintaining stable mental health functioning.
  • Detect patients who are at risk and develop preventive plans to avert the deterioration of their condition.
  • Coordinate the delivery of health services related to behavioral health, ensuring comprehensive care that includes collaboration with external providers when appropriate.
  • Maintain accurate clinical documentation of patient progress, diagnostic findings, and treatment interventions in accordance with best practices and legal requirements.
  • Facilitate open communication with primary care providers to ensure a seamless integration of care, continuously working together to optimize treatment outcomes.
  • Empower patients through self-management strategies and educational resources to actively participate in their treatment and recovery journeys.
  • Provide valuable insights to clinic management regarding behavioral health services and areas for improvement in patient outcomes and program evaluations.
  • Assist patients in adhering to their medical treatment plans by offering strategies to cope with any side effects of prescribed medications.
  • Commit to ethical clinical practices and engage in ongoing professional development to enhance counseling competencies.
Patient Population / Client Focus

The LPC will serve a diverse patient population that includes individuals facing a variety of mental health issues, from anxiety and depression to more complex psychological conditions. Our clinic emphasizes culturally sensitive approaches, and candidates who are bilingual in English and Spanish are particularly encouraged to apply, though this is not a requirement. Qualifications
  • Masterโ€™s Degree in Clinical Counseling or a related field.
  • Current and valid LPC (Licensed Professional Counselor) license in the state of Arizona.
