Job description

  1. SPECIFIC RESPONSIBILITIES
  • Ensure all site-based and hub-based day habilitation programs maintain OPWDD certification and comply with 14 NYCRR §635-10.4 and related standards.
  • Maintain COA accreditation by adhering to documentation, service delivery, and quality assurance protocols.
  • Oversee compliance regarding service documentation and Staff Action Plans.
  • Manage facility contracts and ensure fleet maintenance for all day habilitation vehicles.
  • Supervise assistant supervisors and oversee daily operations at The Center (site-based facility).
  • Coordinate monthly recreation calendars for both site-based and hub-based programs.
  • Review all Staff Action Plans (SAPs) for site-based and Program without Walls (PWW) participants.
  • Provide direction program supervisors to ensure SAPs align with Life Plan goals and OPWDD expectations before submission to the Director of Day Habilitation.
  • Ensure SAPs are distributed to care managers within 60 days of Life Plan review or service start date.
  • Conduct regular meetings with program supervisors to address OPWDD regulations, programmatic issues, and event planning.
  • Collaborate with residential program coordinators to improve communication and integration between residential and day habilitation services.
  • Partner with Program without Walls PWW hub site supervisors to develop and assess meaningful on-site activities for individuals supported.
  • Attend SCO leadership and IRC meetings in the absence of the Director.
  • Participate in IDT meetings for day habilitation participants.
  • Serve as a liaison to families, ensuring satisfaction and quality of services.
  • Be available 24/7 to address participant and program-related emergencies.
  • Collaborate with the Director to develop and uphold day habilitation policies and procedures.
  • Works in tandem with Director of Day Habilitation to establish prevocational program by attending OPWDD virtual meetings and trainings regarding prevocational and vocational training.
  • Maintain SCIP-R certification, CPR First Aid & AED,
  • Maintain AMAP Certification.
  1. QUALIFICATIONS Required, bachelor's degree in human services (master's preferred); strong management, organizational, and interpersonal skills; strong oral and written communication skills; knowledge of OPWDD regulations; minimum of 3 years' experience in OPWDD-funded program with minimum of 2 years in day Habilitation Services. Computer-literate.

The Day Habilitation Program Coordinator oversees the daily operations, compliance, and quality of site-based and hub-based day habilitation services. This role ensures that all programming meets OPWDD regulations, COA accreditation standards, and agency policies, while promoting person-centered planning and community integration
