Comuniife Inc. ย is looking for Program Health Aides for its Medical Respite Program.ย The Program Health Aide position provides assistance in daily living and personal care services to meet patientโ€™s needs. Under the supervision of the Senior Director of Programs, the Program Health Aide will provide routine personalized care. Essential Duties:
  • Observe, monitor and record patient condition. Including physical and mental condition, exercise, as well as intake and outputย 
  • Notify care team and clinical supervisor of changing or unusual conditionsย 
  • Assist in personal care by maintaining hygiene regimens such as bathing, toileting, dressing and groomingย 
  • Support patients by providing housekeeping, laundry services, shopping for food, preparing and serving meals, running errands and other household requirementsย 
  • Educate patients on how to care for themselves by teaching them how to properly use a cane/ walker, self-administration of medication, special utensils to eat and equipment for personal hygiene, as necessaryย 
  • Maintain and keep patients living area clean and orderlyย 
  • Advise on nutrition. Follow prescribed dietary requirements and nutrition standardsย ย 
  • Follow plan of care and assist in exerciseย 
  • Assist in the escorting of patients outside of the facilityย 
  • Perform range of motion and other simple extensional therapy servicesย 
  • Identify new recreational opportunities or activities to help patients remain activeย 
  • Maintain strong working relationships with patient to maintain trustย 
  • Maintain a safe, secure and healthy patient environment by following security precautionsย 
  • Adhere to Comunilife professional standards, home care policies and procedures

Education and Experience Requirement(s):
  • High School Diploma or GEDย 
  • CPR, First Aid and Crisis Prevention certificationย 
  • At least one-year experience providing home health aide services. Experience supporting individuals with disabilities, mental illnesses or challenging behaviorsย 
  • At least one-year experience with utilizing electronic medical record systemsย 
  • One year of relevant community-based residential management or social service agency experienceย 
  • Bilingual in Spanish preferred
